A medical home is a model for patient care that encourages the patient’s care team to work together to meet common goals. There are many benefits to being part of our Patient Centered Medical Home (PCMH) program including a secure web-based patient portal where you can communicate with your doctors office and have e-visits, and your own personal non-clinical case manager who will help assess your needs and encourage you in your ultimate goal of increased health.
